Please note:

• This application is for first-time cardholders.
• This application is for Fayette County residents ages 17 and younger.  It must be completed by a parent or legal guardian who is a Fayette County resident ages 18 and older.
• Please provide your legal name as listed on official forms of identification.
• If your child is eligible for a library card but you can't complete this application successfully, you may apply in person at any Lexington Public Library location.
