The Diabetes Self Management
Program (DSMP) is a peer-led workshop for individuals with a Type II
diabetes diagnosis.

DSMP workshops are designed to help individuals gain self-confidence in
their ability to control their symptoms and learn how their health
problems affect their lives. Small-group, highly interactive workshops
are six (6) weeks long, meeting once per week, and are facilitated by a
pair of leaders of which one or both are individuals with Type II
diabetes.

The workshop topics include the following:

. Working with Your Health Care Professional and Health Care System
. Preventing Low Blood Sugar
. Relaxation Techniques and Stress Management
. Monitoring Blood Sugar
. Preventing Complications.

If you have questions regarding your Type II diabetes diagnosis, or are
struggling with handing this illness, join us for this free workshop. Registration required.

research that shows the numerous benefits it brings. Come experience for
yourself ways to practice mindfulness in both stillness (sitting practices)
and movement (gentle yoga and walking).  No experience is
necessary. This program will meet for 3 consecutive Thursday mornings,October 1, 8 and 15 from 10-11 AM. Registration required.

This presentation is designed to help those who are new to Medicare and give caregivers a better understanding of the Medicare program. Even those who currently have Medicare coverage could benefit from this detailed overview. Topics will include an introduction to Medicare including what Medicare covers, supplemental Medicare Health plans including Medicare Supplements, Medicare Advantage plans, and Part D prescription drug coverage. All registered attendees will receive the most updated Medicare guide booklet as well. (This an educational presentation)

This session is a 60-minute presentation that includes time for scenarios and Q&A.

conditions, improve their quality of life, and lower health care costs.
CDSMP workshops are designed to help people gain self-confidence in
their ability to control their symptoms and learn how their health
problems affect their lives. Small-group, highly interactive workshops
are six weeks long, meeting once a week, and are facilitated by a pair
of leaders, one or both of whom are non-health professionals with
chronic diseases themselves.
The workshop topics include the following:
. How to deal with frustration, fatigue, pain, and isolation
. Ways to maintain and improve strength, flexibility, and endurance
. Managing medications
. How to communicate more effectively with family, friends, and health
professionals
. Healthy eating
If you're struggling with a chronic condition, join us for this free
workshop! Registration required.
